Eligibility Requirements

 

The Alabama Department of Mental Health, Division of Developmental Disabilities (DD) is the operating agency for Medicaid and assists persons with intellectual and developmental disabilities.  All individuals enrolled at MCCR are served under ID waiver and /or The Living at Home waiver which offers the same services, except residential habitation.  Alabama maintains a state-wide ID waiver waiting list.

To apply for services for a loved one, please call the Call Center at (800) 361-4491 to start the application process and be placed on the ID waiting list.   Eligibility for the ID Disabilities Waiver is determined by an assessment of the individual.  The Waiting List serves the most critical first.  

 

Other eligibility requirements include:

 

1. An I.Q. test result below 70;

2. Significant functional limitations;

3. SSI or Medicaid eligible;

4. Income limitations.
